Chẩn đoán và tiên lượng bệnh nhân nhiễm khuẩn huyết trong giai đoạn sớm đóng vai trò quan trọng, giúp giảm tỷ lệ tử vong, rút ngắn thời gian nằm viện của bệnh nhân. Mục tiêu: Nghiên cứu giá trị tiên lượng của thang điểm SOFA ở bệnh nhân nhiễm khuẩn huyết người lớn. Đối tượng và phương pháp: 110 bệnh nhân trên 15 tuổi được chẩn đoán nhiễm khuẩn huyết, điều trị tại Khoa Bệnh Nhiệt đới và Khoa Hồi sức tích cực, Bệnh viện Trung ương Huế từ tháng 01/2021 - 9/2022. Nghiên cứu cắt ngang. Kết quả: Về tiên lượng diễn biến nặng, SOFA_T0 > 4,5 cho độ nhạy 69,4% và độ đặc hiệu 76,2%; SOFA_T24 >3,5 cho độ nhạy 86,1% và độ đặc hiệu 71,4%; SOFA_T24_T0 > -0,1 cho độ nhạy 80,6 và độ đặc hiệu 52,4%. Về tiên lượng tử vong, SOFA_T0 > 2,5 cho độ nhạy 90,0% và độ đặc hiệu 45,6%; SOFA_ T24 >2,5 cho độ nhạy 90,0% và độ đặc hiệu 51,9%; SOFA_T24_T0 > -0,1 cho độ nhạy 85% và độ đặc hiệu 46,8%. SOFA_T24 có giá trị tiên lượng nặng và tiên lượng tử vong tốt so với SOFA_T0, SOFA_T24_T0 và lactat_T0. Các yếu tố liên quan đến tiên lượng tử vong là thời gian nằm viện, SOFA_T24 và nồng độ creatinin máu với OR lần lượt là 0,874; 2,299 và 1,009. Kết luận: SOFA_T24 có giá trị tiên lượng bệnh diễn biến nặng tốt với điểm cắt > 3,5; độ nhạy 86,1% và độ đặc hiệu 71,4%. SOFA_T24 có giá trị tiên lượng tử vong tốt với điểm cắt > 2,5; độ nhạy 90,0% và độ đặc hiệu 51,9%. Các yếu tố liên quan đến tiên lượng tử vong ở bệnh nhân nhiễm khuẩn huyết là thời gian nằm viện, SOFA_T24 và nồng độ creatinin máu với OR lần lượt là 0,874; 2,299 và 1,009.
Diagnosis and prognosis of sepsis patients in the early stage play an important role, helping to reduce mortality and shorten the hospital stay of patients. Aims: Study prognostic value of SOFA score in sepsis adult patients. Subjects and methods: 110 patients over 15 years old were diagnosed with sepsis, treated at the Department of Tropical Diseases and Intensive Care Unit, Hue Central Hospital during the period from January 2021 to September 2022. A cross-sectional study. Results: In predicting severe disease, SOFA_T0 > 4,5 has the sensitivity 69,4%, specificity 76,2%; SOFA_T24 >3,5 has the sensitivity 86,1%, specificity 71,4%; SOFA_T24_T0 > -0,1 has the sensitivity 80,6%, specificity 52,4%. In the prognosis of mortality, SOFA_T0 > 2,5 has the sensitivity 90,0%, specificity 45,6%; SOFA_T24 > 2,5 has the sensitivity 90,0%, specificity 51,9%; SOFA_T24_T0 > -0,1 has the sensitivity 85,0%, specificity 46,8%. SOFA_T24 has good predictive value for severe prognosis and mortality. The factors that are related to mortality in sepsis patients are time of hospital stay, SOFA score after 24 hours of admission and serum creatinine with OR 0,874; 2,299 and 1,009, respectively.Conclusions: SOFA_T24 had good predictive value of severe disease with cutoff > 3,5 has the sensitivity 86,1% and specificity 71,4%. SOFA_T24 had good predictive value of mortality with cutoff > 2,5 has the sensitivity 90,0% and specificity 51,9%.
