Fascioliasis can cause multiple serious complications, which affect the well-being of the community. Reported complications include cholangitis, biliary stone formation, sclerosing cholangitis, biliary cirrhosis and acute pancreatitis. Among them, hemobilia is a very rare complication. Here, the authors present a case with hemobilia associated with biliary fascioliasis, Which is quite a rare case. A 34 year-old male patient was admitted with upper gastrointestinal hemorrhage, diagnosed with hemobilia from fasciloliasis lesion after c.onsidering his typical clinical manifestations, classic images on ultrasound and CT scan and a positive ELISA for fasciola spp. He was treated with triclabendazole initially. After no significant response observed, hepatectomy was indicated and the patient gradually improved after the surgery. This case reminds us of the importance of fascioliasis as one of the cause of hemobilia (though rare), especially if patient lives endemic regions of fasciola species.
