Abstract

Objective: Evaluating the soon results after bidirectional Glenn shunts (BDGS) operation at the Cardio-vascular Center - E Hospital. Subjects and Methods: the authorsdid a retrospective study of 28 patients who had undergone bidirectional Glenn shunts operation at the Cardio-vascular Center E hospital from 6/2011 - 6/2012. Results: The mean age was 6 + or - 1years (6 months - 21 years). The average weight was 15.6 + or - 2kg (5kg - 43kg). All patients had moderate to severe cyanosis, the average arterial blood oxygen saturation was 71.8 percent + or - 1.5 percent (50 percent - 82 percent). 3 patients had artery-pulmonary bypass before. Cardiopulmonary bypass was used in all patients. 9 patients had bilateral BDGS, 20 patients had BDGS; 3 patients with BlalockTaussig shunt interruption. Postoperative superior vena cava pressure averaged 17.5 + or - 2.6 mmHg (14-24). All patients used postoperative pulmonary vasodilator. Arterial blood oxygen saturation after surgery compared with before surgery: 14 + or - 0.6 percent (statistical significance at p 0.05). Mortality rate was 14.3 percent. The average duration of mechanical ventilation: 49.1 + or - 2.8 hours (0.3 - 72), length of stay in intensive care: 5.6 + or - 1.2 days (2-30), length of hospital stay: 17 + or - 2 days (2-64). 23/28 patients discharged from the hospital with significant improvement in clinical and blood oxygen saturatIon. Discussion: BDGS operation is considered to be intermediate stages in the strategy repaired congenital heart defect with univentricular preparation for Fontan operation. It is done as a first or second stage after artery-pulmonary bypass or pulmonary artery Banding procedure. The study shows that a relatively positive early results for patients with Glenn operation.
