During 2011, three cases of retrobulbar hemorrhage with orbital compartment syndrome, proptosis, increased IOP and acute loss of visual acuity were decompressed by lateral canthotomy and inferior cantholysis in the Trauma Department of Eye Hospital of HCM city. Medical treatment helps decrease IOP, orbital edema and inflammation. The first case with acute loss of visual acuity was performed the emergent orbital decompression less than 2 hours since lower lid blepharoplasty. Vision totally recovered after 5 days of treatment. The second case that showed proptosis, increased IOP weak light perception was decompressed late at 10 days after the blunt trauma. The visual acuity improved very little to the hand movement but increasing ocular tension returned yo normal after lateral canthotomy and inferior cantholysis. The third case of severe orbital hemorrhage caused by hitting against a hard object in the traffic accident. Total visual loss, increased IOP, proptosis, optic nerve damage were documented at the time of 7 hours after accident and hospitalization. This patient was done the orbital decompression quickly. After that, ocular tension returned to normal, proptosis resolved but the vision did not improve due to severe optic nerve damage.
