Abstract

Objectives: describing of the clinical and paraclinical features and surgical outcomes of 93 patients with acute pancreatitis due to gallstones at Viet-Duc Hospital 2007-2010. Patients and Methods. Retrospective descriptive study on the patients with gallstones pancreatitis being operated from 2007-2010 in Viet-Duc Hospital. Results and discussion: The average age: 54.42 + or - 17.67 years (range 19-89). Female: 63 patients (67.7 percent). Farmers: 44 patients (56.4 percent). Prehistory of gallstone surgery: 38 patients (40.9 percent). Pain in the right upper quadrant and epigastric region: 93 patients (100 percent), fever: 53 patients (57 percent), jaundice: 56\ patients (60.2 percent). The abdominal tenderness: 60 patients (64.5 percent). Mayo Robson' sign (+): 33 patients ,(35.5 percent). High serum amylase (more 6 times normal): 62.4 percent. High serum total billirubin: 87.8 percent . Ultrasound: oedematous pancreatitis, 26 patients (28 percent), gallstones: 93 patients (100 percent). E.coli is the most common (59.3 percent). Per operative lesions: CBD stones: '78 patients (83.9 percent). Pancreatic necrosis: 12 patients (17.4 percent). Operations: 78 patients (83.9 percent) were performed choledochotomy for removing biliary stones with T tube (+ or - cholecystectomy); Opened capsule and drainage: 10 patients (10.8 percent). Results: The length of hospitalization: 11.5 + or - 4.61 days (range 3-29 days). Complications: 5 patients (no patients had reintervention). No cases were fatal or serious about applying. Conclusion: gallstones pancreatitis were common in the age 41-60 (39.8 percent). Common symptoms were right upper quadrant and epigastric pain (100 percent), fever (57 percent), jaundice (60.2 percent), abdominal wall defence (64.5 percent), Mayo Robson'sign (+): 35.5 percent. Amylase were higher 6 times (62.4 percent); 17.4 percent had pancreatic necrosis. Choledochotomy accounted for 83.87 percent; 10.8 percent opened capsule and drainage. Low complication rate, no case was reopened and no deaths or serious about applying. Length of hospitalization after surgery: 11.35 + or - 4.61 days (3-29).
