Vạt da cân đùi trước ngoài (ALT) có nhiều ưu điểm trong phẫu thuật tái tạo khuyết gân Achilles và phần mềm che phủ. Nghiên cứu trên 6 bệnh nhân từ 12 đến 62 tuổi, đều có tổn thương khuyết gân Achilles và phần mềm vùng gót với tình trạng nhiễm trùng nặng khi vào viện. Kích thước của khuyết phần mềm từ 6 x 4 cm tới 12 x 10 cm. Sử dụng vạt da cân ALT tự do dạng phức hợp ở 1 trường hợp, dạng chùm ở 5 trường hợp. Các vạt đều có hai thành phần đảo da và fascia lata (FL), kích thước của vạt da từ 10 x 5 cm tới 15 x 12 cm, kích thước vạt FL từ 10 x 5 cm tới 15 x 12 cm. Làm mỏng vạt ALT dưới kính hiển vi và làm chiều dày vạt mỏng tới 40-79% ở 5/6 bệnh nhân. Sau phẫu thuật các bệnh nhân đều đứng, đi lại được và rất hài lòng với kết quả. Vạt da cân ALT tự do sử dụng dưới các dạng khác nhau như vạt phức hợp, vạt chùm với hai thành phần chính là vạt da và vạt FL là sự lựa chọn thích hợp cho các khuyết lớn vùng gân Achilles và phần mềm trong một lần phẫu thuât. Kỹ thuật làm mỏng vạt da được kết hợp với vạt chùm cho kết quả thẩm mỹ cao hơn.
The free anterolateral thigh flap (ALT) with fascia lata (FL) shows advantages in the singlestage reconstruction of the Achilles tendon and soft tissue defect. We performed on a series of 6 patients from 12 - 62 years old, receiving ALT flaps with FL to reconstruct the Achilles tendon and soft tissue defects with infected areas. The skin defect sizes are from 6 x 4 cm to 12 x 10 cm. One patient underwent reconstruction using the free composite ALT flap with vascularized FL, while other 5 patients underwent reconstruction using the free chimeric ALT and FL flaps. Flaps have 2 components: cutaneous island and fascial flap. The size of the cutaneous island ranged from 10 x 5 cm to 15 x 12 cm, and the size of the FL flap ranged from 10 x 5 cm to 15 x 12 cm. Five patients received skin flap thinning up to 40 - 79% of the flap thickness, while one patient did not need to debulk. All patients were able to stand and ambulate, and they were satisfied with the reconstructive results. The thinned chimeric ALT with FL flap is the best choice for the massive Achilles tendon and overlying skin defect due to its many advantages. We believe that this is a practical approach to improve the functional outcomes of patients with massive Achilles tendon and overlying skin defects.
