Đánh giá kết quả chăm sóc hệ thống tưới rửa kháng sinh cho bệnh nhi sau phẫu thuật viêm xương tủy theo đường máu tại khoa Chỉnh hình Nhi, Bệnh viện Nhi Trung ương. Nghiên cứu mô tả tiến cứu trên 54 bệnh nhi viêm xương tủy theo đường máu được phẫu thuật theo phương pháp khoan xương, đặt hệ thống tưới rửa kháng sinh tại khoa Chỉnh hình Nhi, Bệnh viện Nhi Trung ương từ tháng 6 năm 2019 đến tháng 8 năm 2020. Thường gặp ở trẻ từ >30 ngày đến < 5 tuổi chiếm 53,7 %. Tỷ lệ trẻ trai / trẻ gái = 1/1. Tỷ lệ mắc bệnh ở nông thôn (61,1%) cao hơn ở thành thị (38,9%). Triệu chứng sốt Trước tưới rửa 77,8%, ngày thứ 3 còn 48,1%, ngày thứ 7 còn 3,7%. Đau Trước tưới rửa 98,1%, ngày thứ 3 còn 50%, ngày thứ 7 còn 1,9%. Sưng nề Trước tưới rửa 94,7%, ngày thứ 3 còn 38,9%, ngày thứ 7 còn 5,6%. Số lượng bạch cầu bình thường Trước tưới rửa chiếm 20,4%, ngày thứ 3 chiếm 24,1%, ngày thứ 7 chiếm 57,4%. Số lượng bạch cầu trung tính bình thường Trước tưới rửa chiếm 38,9%, ngày thứ 3 chiếm 55,6%, ngày thứ 7 chiếm 75,9%. Phản ứng CRP bình thường Trước tưới rửa chiếm 1,9%, ngày thứ 3 chiếm 1,9%, ngày thứ 7 chiếm 29,6%. Nuôi cấy dịch rửa Trước tưới rửa dương tính 94,4%, ngày thứ 3 chiếm 61,1%, ngày thứ 7 chiếm 31,5%. Trong 54 bệnh nhi có 18 trường hợp tắc 16,7% tắc đầu vào, 83,3% tắc đầu ra. Tắc do gập dẫn lưu chiếm 66,7%, do cục máu đông chiếm 22,2%, do hết dịch rửa chiếm 11,1%.
To evaluate the results of the Continuous Postoperative Antibiotic Irrigation (CPAI) for acute hematogenous osteomyelitis (AHO) children at the Pediatric Orthopedics Department, Vietnam National Children’s Hospital. A descriptive research was conducted on 54 AHO children operated by the cortical bone fenestrations with CPAI at the Pediatric Orthopedics Department, Vietnam National Children’s Hospital from June 2019 to August 2020. Common in children from> 30 days to <5 years old accounts for 53.7%. Ratio of boys / girls = 1/1. The prevalence rate in rural areas (61.1%) was higher than that in urban areas (38.9%). Fever symptoms Before irrigation 77.8%, on the 3rd day 48.1%, on the 7th day 3.7%. Pain 98.1% before irrigation, 50% on day 3, and 1.9% on day 7. Swelling 94.7% before irrigation, 38.9% on the 3rd day, and 5.6% on the 7th day. Normal leukocyte count Before irrigation accounted for 20.4%, accounted for 24.1% on the 3rd day, and on 7th day accounted for 57.4%. Number of normal neutrophils 38.9% before irrigation, 55.6% on the 3rd day, and 75.9% on the 7th day. The normal CRP response Before irrigation, it accounted for 1.9%, on the third day accounted for 1.9%, and on the seventh day accounted for 29.6%. Washing liquid culture Before irrigation, it was 94.4% positive, on the 3rd day it accounted for 61.1%, and on the 7th day it was 31.5%. In 54 pediatric patients, there were 18 cases of obstruction 16.7% of input obstruction, 83.3% of output obstruction. Obstruction due to fold drainage accounted for 66.7%, due to blood clots accounted for 22.2%, due to the absence of wash fluid accounted for 11.1%.Through applying CPAI in 54 children with acute hematogenous osteomyelitis, we found that the clinical and subclinical characteristics positively change from day 3 to day 7
