Gãy thân xương cẳng chân là loại thương tổn thường gặp phổ biến, chiếm tỷ lệ 18% các loại gãy xương và gặp phổ biến ở lứa tuổi lao động. Sử dụng kỹ thuật kết hợp xương bằng đinh chốt nội tủy có chốt điều trị có nhiều ưu điểm. Ở bệnh viện chúng tôi, điều trị gãy thân xương cẳng chân bằng đinh chốt nội tủy có chốt đã được tiến hành trong nhiều năm nay. Để đánh giá kết quả đã làm chúng tôi tiến hành nghiên cứu để tài “Đánh giá kết điều trị gãy kín thân xương cẳng chân bằng nắn kín, đóng đinh nội tủy có chốt ở người lớn tại Bệnh viện đa khoa thống nhất Đồng Nai”. Mục tiêu: Mục tiêu tổng quát: Đánh giá kết quả điều trị gãy kín thân xương cẳng chân bằng nắn kín, đóng đinh nội tủy có chốt kín dưới màn tăng sáng; Mục tiêu cụ thể: Xác định tỉ lệ tai biến, biến chứng của phẫu thuật đóng đinh nội tủy xương cẳng chân; Kết quả điều trị nắn chỉnh ổ gãy và phục hồi chức năng; Mối liên quan: Kết quả nắn chỉnh ổ gãy với độ gãy xương và kết quả phục hồi chức năng với thời gian phẫu thuật. Phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang Kết quả nghiên cứu: Chúng tôi đã phẫu thuật và theo dõi được 35 trường hợp. Với kết quả tỉ lệ rất tốt gặp 26 ca (74,3%), tốt gặp 09 ca (25,7%), không có kết quả trung bình và xấu. Không gặp trường hợp nào bị tai biến, nhiễm trùng nông sau mổ chiếm 8,6%, không có trường hợp nào nhiễm trùng sâu, viêm rò kéo dài. Kết luận: Nhiễm trùng nông sau mổ chiếm 8,6%, không có trường hợp nào nhiễm trùng sâu, viêm rò kéo dài.Lứa tuổi 18-60 chiếm đa số 88,5%. Tai nạn giao thông là nguyên nhân chủ yếu 82,6%. Gãy kín vị trí 1/3 giữa hay gặp chiếm 71,4% với đường gãy loại A chiếm tỉ lệ 85,7%, thời gian phẫu thuật dưới 60 phút tỉ lệ khoảng 88,5%, tỉ lệ phục hồi rất tốt 74,3%. Kết quả nghiên cứu cho thấy có sự liên quan giữa độ gãy xương với kết quả nắn chỉnh ổ gãy với chỉ số Fisher’s exact 0.014. Và cũng có sự liên quan giữa thời gian với phẫu thuật với kết quả phục hồi chức năng với Fisher’s exact 0,0001.
Fractures of lower leg bones are a common type of injury, accounting for 18% of all bone fractures and are common in the working age group. Bone fixation technique with intramedullary nails has many advantages. At our hospital, surgical treatment of two lower leg bones fractures with intramedullary locking nails has been performed for many years. To evaluate the results of this treatment method, we conducted a reseach “The results of treatment of closed lower leg bones fractures with closed reduction, intramedullary locking nails at thong nhat dong nai general hospital”. Objectives: General objective: To evaluate the results of closed lower leg bones fractures treatment with closed reduction and intramedullary locking nails under a brightening screen; Specific goals: To determine the rate of complications of surgical treatment with intramedullary nailing in the lower leg; Results of fracture reduction and rehabilitation; Relationship: Between results of fracture reduction and fracture severity; between results of rehabilitation and surgical time. Research method: Cross-sectional descriptive studyResearch results: We operated and followed up 35 cases. With very good results, there were 26 cases (74.3%), good cases in 09 cases (25.7%), and no average or bad results. There were no cases of complications, superficial post-operative infection accounted for 8.6%, no cases of deep infection or prolonged fistula inflammation. Conclusion: Superficial infection after surgery accounts for 8.6%, there are no cases of deep infection or prolonged fistula inflammation. Ages 18-60 account for the majority of 88.5%. Traffic accidents are the main cause of 82.6%. Closed fractures in the middle third are common, accounting for 71.4%, with type A fractures accounting for 85.7%, surgery time less than 60 minutes, approximately 88.5%, and very good recovery rate 74.3%. Research results show that there is a relationship between fracture severity and fracture reduction results with Fisher’s exact index of 0.014.
