The objective of this study was to describe the a new kind of expanded flap to evaluate the clinical aspects in recontruction of scalp major defects with this new flap. The results show that 19 tissue expandors, 26 flaps in flap were successfully perfomed, partial necrosis was in two flaps (11.1 percent). The functional and aesthetic outcomes were considerated good in 88.9 percent of patients. 24 flaps survived on less rotation and large pedicle. In conclusions, flap in expanded flap specially is designed to reconstruct the scalp major defects. There are many advantages of this flap: high survival of the flap in new position, maximising gain of the skin expansion, favourable change of hair direction.
