Đánh giá kết quả bước đầu của sinh thiết màng hoạt dịch (MHD) dưới hướng dẫn siêu âm trong chẩn đoán một số bệnh lý khớp háng. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang các bệnh nhân có chỉ định và được sinh thiết MHD khớp háng dưới hướng dẫn siêu âm. Mẫu bệnh phẩm sinh thiết sẽ được đánh giá về mô bệnh học, cấy vi khuẩn và xét nghiệm PCR lao. Kết quả: Từ tháng 09/2020 đến tháng 10/2022, có 17 bệnh nhân có chỉ định và được sinh thiết MHD khớp háng dưới hướng dẫn siêu âm tại bệnh viện Đại học Y Hà nội. Có 1 trường hợp không lấy được bệnh phẩm. Số còn lại có 1 bệnh nhân u MHD, 3 bệnh nhân NK sinh mủ, 2 bệnh nhân lao khớp và 10 bệnh nhân viêm MHD không do nhiễm khuẩn. Không có biến chứng xảy ra trong hoặc sau khi sinh thiết. Kết luận: Sinh thiết MHD khớp háng dưới hướng dẫn siêu âm là phương pháp tương đối dễ thực hiện, an toàn, có hiệu quả chẩn đoán cao trong trường hợp các phương pháp khác chẩn đoán chưa rõ ràng.
To evaluate the initial results of ultrasound - guided synovial biopsy in the diagnosis of several coxofemoral joint diseases. Matherial and Method: The cross sectional descriptive study on the patients who had the indication and underwent the coxo-femoral joint ultrasound guided synovial biopsy. The biopsy samples were analyzed in the pathology, bacterial culture and tuberculosis PCR. Results: From 09/2020 to 10/2022, seventeen patients who had the indication and underwent the coxo-femoral ultrasound guided synovial biopsy at Hanoi Medical University Hospital. Among them, one patient was failed to get biopsy sample, three patients suffered a bacterial infection, two patients had a tuberculosis and 10 patients had non-bacterial inflammation. There was no important complication and/or hemorrhage in/after synovial biopsy procedure. Conclusion: The coxo-femoral ultrasound-guided synovial biopsy was a safe, relatively simpliste and high effective method for the diagnosis of several coxo-femoral joint diseases when the other methods could not provide the clear diagnosis.
