Polydactyly is the most common hand anomaly among the congenital anomalies that affect the upper extremity. The thumb polydactyly has multiple anatomical alterations of the skin, soft tissue including nail, tendons, bone and joints, which not only impact the hand function, the cosmetic appearance of hand, but also greatly bear a psychosocial effects. The study was conducted on sixty-five patients (fourty-six male and nineteen-female) with sixty-eight thumb polydactyly. The patients were assessed by both clinical examining and X-ray imaging in order to identify the alteration in anatomy and thumb function. The data showed that there is a high prevalence of right thumb polydactyly. (61.5 percent). The radial polydactyly was small, the miss-alignment was great (mean of 38.3 0) and the mobility was limited. According to Islam classification, there was no patients with type I whilst type IV was the most common (52.4 percent). In conclusion, the morphological characteristics of the thumb polydactyly are variable with multiple different classifications.
