Nhận xét kỹ năng sử dụng dụng cụ phân phối thuốc của người bệnh COPD tại Bệnh viện Đa khoa tỉnh Nam Định. Đối tượng nghiên cứu: 82 người bệnh được chẩn đoán Đợt cấp COPD đã điều trị ổn định tại khoa nội tổng hợp, Bệnh viện Đa khoa tỉnh Nam Định năm 2021. Phương pháp nghiên cứu: mô tả tiến cứu. Kết quả: tuổi trung bình 72 ± 10,672, nam/ nữ xấp xỉ 3,3/1; tỉ lệ bệnh nhân GOLD D cao nhất 45,1%. Kỹ năng sử dụng bình xịt định liều: 73,2% không thao tác bước thở ra hết sức; làm sai bước phối hợp tay bấm miệng hít đồng thời hít vào chậm sâu dài (75,6%); bước nín thở 10s sau xịt làm sai (42,7%). Kỹ năng sử dụng bình hít bột khô: 53,7% không thao tác bước thở ra hết sức; bước nín thở 10s sau đó thở ra từ từ sai (46,3%). Kết luận: thực hành sử dụng các dụng cụ phân phối thuốc của người bệnh COPD còn nhiều sai sót, chủ yếu ở các bước thở ra hết sức trước khi hít thuốc hoặc nín thở 10s sau đó từ từ thở ra, động tác phối hợp tay bấm miệng hít làm giảm hiệu quả điều trị bệnh.
Assessing the skill of using drug dispensing equipment of COPD patients at Nam Dinh Provincial General Hospital. Subjects: 82 patients diagnosed with COPD exacerbation were stabilized at the general internal medicine department, Nam Dinh General Hospital in 2021. Research method: prospective description. Results: mean age 72 ± 10,672, male/female approximately 3.3/1; the highest percentage of GOLD D patients was 45.1%. Skill in using metered-dose inhalers: 73.2% did not manipulate the full exhalation step; wrong step in hand-pressing-mouth-breathing and inhaling slowly and deeply (75.6%); the step of holding your breath for 10s after spraying is wrong (42.7%). Skill in using a dry powder inhaler: 53.7% did not perform the full exhalation step; hold your breath for 10s, then exhale slowly wrongly (46.3%). Conclusion: There are many errors in the practice of using drug delivery devices by patients with COPD, mainly in the steps of exhaling as hard as possible before inhaling the drug or holding the breath for 10 seconds and then slowly exhaling, coordinated hand-to-mouth movements. Inhalation reduces the effectiveness of treatment.
