So sánh hiệu quả dự phòng run của ketamin 0,2mg/kg và ondansetron 4mg tiêm tĩnh mạch sau gây tê tuỷ sống mổ lấy thai. Đối tượng và phương pháp nghiên cứu: Nghiên cứu tiến cứu, thử nghiệm lâm sàng ngẫu nghiên có so sánh giữa 2 nhóm: 50 bệnh nhân được tiêm tĩnh mạch ketamin 0,2mg/kg (nhóm K) và 50 bệnh nhân được tiêm tĩnh mạch ondansetron 4mg (nhóm O), tại bệnh viện Quân y 354, từ tháng 12/2022 đến tháng 06/2023. Kết quả: Tỷ lệ bệnh nhân run trong mổ ở nhóm O là 34%; nhóm K là 14%, khác biệt có ý nghĩa thống kê. Tỉ lệ bệnh nhân run sau mổ nhóm O là 8%; nhóm K là 2%. Thời gian BN bị run trong mổ nhóm O là 12,8±3,5 phút; nhóm K là 14,1±4,6 phút. Thời gian BN bị run sau mổ nhóm O là 12,5±2,9; nhóm K là 10 phút. Mức độ run của BN trong mổ ở nhóm O là 23,5% độ 2; 76,5% độ 3; ở nhóm K là 42,9% độ 2; 57,1% độ 3. Mức độ run của BN sau mổ ở nhóm O là 100% độ 2; ở nhóm K là 100% độ 1. Tất cả bệnh nhân run đều xuất hiện trong 20 phút đầu sau GTTS, chủ yếu là 10 phút đầu. Kết luận: Ketamin và ondansetron có hiệu quả tốt trong dự phòng run sau gây tê tuỷ sống mổ lấy thai.
Comparison of the efficacy of shivering prophylaxis between intravenous ketamine 0.2 mg/kg and ondansetron 4 mg after spinal anesthesia for cesarean section. Subjects and methods: A prospective, randomized clinical trial was conducted comparing two groups: 50 patients who received intravenous ketamine 0.2 mg/kg (group K) and 50 patients who received intravenous ondansetron 4 mg (group O), at Military Hospital 354, from December 2022 to June 2023. Results: The rate of intraoperative shivering in group O was 34%, while in group K it was 14%, a statistically significant difference. The rate of postoperative shivering in group O was 8%, while in group K it was 2%. The duration of intraoperative shivering in group O was 12.8 ± 3.5 minutes, compared to 14.1 ± 4.6 minutes in group K. The duration of postoperative shivering in group O was 12.5 ± 2.9 minutes, while it was 10 minutes in group K. The severity of intraoperative shivering in group O was 23.5% at grade 2 and 76.5% at grade 3, while in group K it was 42.9% at grade 2 and 57.1% at grade 3. Postoperative shivering severity in group O was 100% at grade 2, while in group K it was 100% at grade 1. All patients who experienced shivering did so within the first 20 minutes after spinal anesthesia, primarily within the first 10 minutes. Conclusion: Ketamine and ondansetron are effective in preventing shivering after spinal anesthesia for cesarean section.
