The classic anterior approach for Congenital Aural Atresia (CAA) used to be narrow approach for creating a new external auditory canal. Objectives: To examine the possibility and the outcome of the posterior transantro-attic approach for surgical corection of the atretic external auditory canal as well as to correct the malformation of the ossides chain. Methods: Prospective descriptive study of 18 casees CAA performed with the posterior transmastold approach for creation of a new external ear canal. Results: Following the initial surgery with this technique, good result was obtained in 14 cases. 4 cases were unsuccessful with stenosis of the newly formed canal, wich required revision surgery in one cases. Results and conclusions: Transantro-attic posterior approach for surgery of CAA is a safe technique which provides a large enough field for surgical correction of the atretic external auditory canal and of the osscular chain malformation.
