Mô tả thực trạng bàn giao người bệnh theo mô hình SBAR của điều dưỡng tại bệnh viện Vinmec Times City. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang được thực hiện qua quan sát 304 ca bàn giao người bệnh của điều dưỡng theo mô hình SBAR tại các khoa lâm sàng Nội, Ngoại, Sản, Nhi, Hồi sức cấp cứu của bệnh viện Vinmec Times City. Kết quả: Tỷ lệ tuân thủ thực hiện bàn giao người bệnh theo mô hình SBAR bằng lời nói thay đổi theo từng nội dung (cấu phần) của mô hình, cụ thể: tuân thủ bàn giao tình huống, diễn biến, đánh giá và đề nghị lần lượt là 69,4%, 40,5%, 94,7%, và 89,5%. Tỷ lệ tuân thủ chung chỉ đạt 29.9%. Tỷ lệ tuân thủ thực hiện bàn giao người bệnh theo mô hình SBAR qua ghi hồ sơ với từng nội dung như sau: tình huống (97%), diễn biến người bệnh (98%), đánh giá (97,4%), đề nghị (98,4%). Tỷ lệ tuân thủ chung bàn giao người bệnh qua ghi hồ sơ đạt 95.1%. Kết luận: Điều dưỡng tuân thủ thực hiện bàn giao người bệnh theo mô hình SBAR qua ghi hồ sơ tốt hơn bàn giao bằng lời (95,1% vs 29,9%). Trong thực hành chăm sóc, người điều dưỡng cần tăng cường giao tiếp và tuân thủ bàn giao người bệnh theo mô hình SBAR bằng lời nói. Cần tăng cường đào tạo và giám sát điều dưỡng thực hiện bàn giao người bệnh theo mô hình SBAR bằng lời nói để nâng cao chất lượng bàn giao qua đó thúc đẩy sự an toàn của người bệnh.
To describe current situation of patient handover based on SBAR model of nurses at the Vinmec Times City Hospital. Methodology: A cross-sectional descriptive study was conducted by observation of 304 patient handover cases implemented by nurses according to the SBAR model in the clinical departments including: Internal Medicine, Surgery, Obstetrics, Pediatrics and Emergency department. Results: The rate of compliance with patient handover verbally according SBAR model varies among components of the model, specifically: compliance with handover of situations, background, assessment and recommendations were 69.4%, 40.5%, 94.7%, and 89.5% respectively. The overall compliance rate was only 29.9%. The compliance rate of patient handover according to the SBAR model through documenting with each component as following: situation (97%), background (98%), assessment (97.4%) and recommendation (98.4%). The overall compliance rate of patient handover through documenting reached 95.1%. Conclusions: Nurses performed patient handover according to the SBAR model through documenting better than verbal handover (95.1% vs 29.9%). In practice, nurses have to strengthen communication and pay close attention to compliance when handing over patients according to the SBAR model verbally. It is necessary to strengthen training and supervision of nurses to hand over patients according to the SBAR model verbally to improve handover quality, thereby enhancing patient safety.
