Mô tả đặc điểm lâm sàng bệnh Coats tại Bệnh viện Mắt Trung Ương. Đối tượng và phương pháp nghiên cứu: nghiên cứu mô tả chùm ca bệnh trên 30 bệnh nhân Coats tại Bệnh viện Mắt Trung Ương từ 6/2021 đến 9/2022. Kết quả: Tuổi trung bình phát hiện bệnh là 8.53 ± 5.36 với 29 nam và 1 nữ. Tỉ lệ bị bệnh ở mắt phải là 43%, mắt trái là 53% và có một bệnh nhân bị cả hai mắt. Triệu chứng cơ năng phổ biến nhất là nhìn mờ (90%), sau đó là lác (43%) và ánh đồng tử vàng (36%). Thị lực chỉnh kính tối đa trung bình theo logMAR là 1.9±1.06. Giãn mạch máu phía thái dương phổ biến nhất với 80% mắt, ít gặp nhất ở phía mũi với 19% mắt. Phạm vi trung bình của mạch máu giãn là 7.00 ± 2.81 cung giờ. Bong võng mạc qua hoàng điểm xuất hiện ở 45% mắt và phù hoàng điểm ở 42% mắt. Giai đoạn 2B phổ biến nhất với 10 mắt, giai đoạn 2A ít nhất với 1 mắt. Tuổi phát hiện bệnh của nhóm bệnh nhân giai đoạn 2A đến 3A là 10.63 ± 6.11 tuổi, giai đoạn 3B đến 5 là 6.47 ± 3.20. Kết luận: Bệnh Coats thường gặp một bên mắt ở trẻ nam với tổn thương điển hình là giãn mạch máu ngoại vi kèm xuất tiết chủ yếu ở phía thái dương. Giai đoạn phổ biến trong nghiên cứu là 2B. Bệnh nhân khởi phát bệnh khi còn nhỏ thường biểu hiện nặng hơn.
To present clinical characteristics of Coats disease at Vietnam National Institute of Ophthalmology. Methods: Descriptive study based on case series was performed of 30 patients diagnosed with Coats disease at Vietnam National Institute of Ophthalmology from June 2021 to September 2022. Results: The mean age of patients at diagnosis was 9.13 ± 5.38 with 29 males and 1 female. The rate of diseased eyes was 43.3% in the right eye, 53.3% in the left eye and one patient had disease in both eyes. The most common symptom was decreased visual acuity (90%) followed by strabismus (43%) and then leukocoria (36%). Mean corrected visual acuity was 1.9±1.06 logMAR. Retinal telangiectasia was most commonly involved temporal quadrant in 80% of eyes while just involved nasal quadrant in only 20% of eyes. The mean range of retinal telangiectasia was 7.00 ± 2.81 clock hours. Macular-off retinal detachment was present in 45% of eyes and macular edema was present in 42% of eyes. Stage 2B was the most common with 10 eyes while there was only 1 eye at stage 2A. The mean age of patients with diagnosis at stage 2A to stage 3A was 10.63 ± 6.11, at stage 3B to stage 5 was 6.47 ± 3.20. Conclusion: Coats disease commonly occurs unilaterally in young males. Typical disorders are peripheral telangiectasia and temporal retinal exudation. The common stage in the study is 2B. Early-onset patients often have more severe symptoms.
