Trình bày kỹ thuật, kết qủa của phẫu thuật tạo hình thành bụng kết hợp hút mỡ có bảo tồn mạch xuyên cấp máu cho vạt da thành bụng, những tai biến biến chứng và cách xử lý. Đối tượng - Phương pháp: Từ tháng 1/2022 đến tháng 1/2024 có 46 bệnh nhân nữ tuổi từ 32 đến 55 có tình trạng tích mỡ dày thành bụng, thừa da, nhão cân cơ thành bụng sau sinh ở các mức độ khác nhau được can thiệp bằng phẫu thuật tạo hình thành bụng toàn bộ kết hợp với hút mỡ có bảo tồn mạch xuyên. Phương pháp tiến cứu can thiệp lâm sàng, theo dõi dọc. Kết quả: Kết quả gần đạt tốt cao 93,4%, kết quả xa đạt tốt 81,4%. Có 4 ca tai biến biến chứng nhẹ (8,7%). Kết luận: Tạo hình thành bụng có thể kết hợp hút mỡ an toàn với kỹ thuật bảo tồn ít nhất một nhánh mạch xuyên cấp máu cho vạt da trên rốn. Kỹ thuật hút mỡ triệt để giúp cho vạt da di trượt dễ dàng hơn, cần chú ý tránh để căng kéo, đứt cuống mạch xuyên khi di chuyển vạt da.
To present the technique and results of abdominoplasty combined with liposuction with preservation of perforators, complications and how to handle them. Subjects - Methods: From January 2022 to January 2024, there were 46 female patients aged 32 to 55 with thick fat accumulation in the abdominal wall, excess skin, and loose fascia of the abdominal wall after giving birth with different degrees. All of them under gone total abdominoplasty combined with liposuction with preservation of perforating vessels. Prospective method of clinical intervention, longitudinal follow-up. Results: Short term results were good at 93.4%, long term results were good at 81.4%. There were 4 cases of minor complications (8.7%). Conclusion: Abdominoplasty can be safely combined with liposuction with a technique that preserves at least one perforating vessel branch supplying blood to the skin flap. Thorough liposuction technique helps the skin flap move and slide more easily. Care should be taken to avoid stretching or breaking the perforator pedicle when moving the skin flap.
