
Objective: description characters of patients with mechanical heart valves use oral anticoagulant after operation. Comparison on Wafarin and Acenocoumarol about correct dosage and efficacy. Method: cross-sectional, observational retrospective comparative study. Results: characteriztion of patients: 56,7 percent of wafarin group and 70 percent of acenocoumarol group had unoptimal theurapeutic range, averaged dose of warfarin group 1,4 + or - 0,53(mg) and of aceocoumarol group 1,6 + or - 1,06(mg) before study, about 50 percent the patients of both groups (before use warfarin) had anticoagulant-related-complication: 6,7 percent mild bleeding, 28,3 percent severe bleeding, 3,3 percent thrombus. About correct dosage and efficacy of Wafarin and Aceno-coumarol: the number of test INR about 4times/patient. Averaged interval time test INR: 3days. Amount correct dose/patient: 2 times. Correct dose percentage of the number of test INR ( percent) in warfarin group(36,37 percent:I: 24,84) less than in acenocoumarol(47,03 percent:1: 34,15) with p0,05. The relationship of warfarin maintenance dose acenocoumarol dose x 0.85 - 1,75 with r 0.3. Quantity in-hospital day: 10days. To come out of hospital: 80,0 percent patients of warfarin group had INR optimal theurapeutic range better than acenocoumarol group (33,3 percent). percentage of the number of INR optimal theurapeutic range (percent) in warfarin group (50,83 percent + or - 28,24) better than in acenocoumarol group (27,3 percent + or - 25,56) with p0,05. Conclution: initial use warfarin show that: it better than acenocoumarol about correct dose and percentage of complication.
