Objectives of this study was to evaluate the vitality of the pedicled anterolateral thigh flap in phalloplasty and to find the element of risks causing necrotizing flap. This is an uncontrolled trial. 28 penile squamous cell carcinoma patients were amputated, then neophalloplasty have been reconstructed by pedicled anterolateral thigh flap. The results showed that 19/28 (67.9 percent) of the flaps survived completely, partially necrotizing flap occured in 7/28 (25 percent), totally necrotizing flap accounted for 2/28 (7.1 percent). Surviable flaps were affected by the combination with dissecting flap, quality of perforator, and swelling flap. In conclusion, the pedicled anterolateral thigh flap is suitable for penile reconstruction.
