Ghép tim - thận đồng thì là dạng ghép đa tạng khá phổ biến, được coi như giải pháp tối ưu cho các hội chứng tim - thận giai đoạn cuối. Báo cáo này thông báo lâm sàng ca ghép tim - thận thành công đầu tiên tại Việt Nam. Bệnh nhân nam 37 tuổi, thời gian mắc bệnh gần 10 năm: suy tim giai đoạn cuối do bệnh cơ tim giãn - chức năng tâm thu thất trái 15-25%, kèm suy thận mạn tính đang chạy thận chu kỳ cách ngày - do bệnh thận IgA. Bệnh nhân được ghép tim và thận đồng thì từ người cho đa tạng chết não ngày 15/02/2023 tại bệnh viện Hữu nghị Việt Đức. Sau mổ diễn biến thuận lợi, chức năng tim và thận ghép phục hồi hoàn toàn sau 5 ngày. Đây là ca ghép đa tạng thành công thứ 2 ở bệnh viện Việt Đức, và là ca ghép tim – thận đầu tiên.
Simultaneous heart and kidney transplantation is quite a popular type of multiorgan transplant in the world, considered an optimum treatment for end stage cardiorenal syndrome. This report presents first successful simultaneous heart-kidney transplant in Vietnam. A 37-year-old male patient with 10 years of chronic disease: end-stage heart failure due to dilated cardiomyopathy with 15-25% of left ventricular systolic function, and renal failure due to IgA nephropathy completely depended on dialysis. The patient underwent a simultaneous heart-kidney transplant from a brain-dead multiorgan donor on February 15, 2023 at Viet Due University hospital. He had quick recovery of all cardiac and renal functions on 5 days afters transplant. This is the 2nd successful case of multiorgan transplant at Viet Due hospital, but the first heart-kidney transplantation.
