Phân tích một số yếu tố ảnh hưởng đến chi phí điều trị nội trú của người bệnh viêm phổi tại Bệnh viện Phạm Ngọc Thạch TP Hồ Chí Minh năm 2023. Phương pháp: Nghiên cứu định tính. Công cụ thực hiện cấu phần định tính gồm bản hướng dẫn phỏng vấn sâu và thảo luận nhóm cho từng nhóm đối tượng về các yếu tố ảnh hưởng đến chi phí của người bệnh viêm phổi đang được điều trị tại bệnh viện, được xây dựng dựa theo khung lý thuyết nghiên cứu sử dụng để phỏng vấn người bệnh/người nhà người bệnh, các bác sĩ điều trị, lãnh đạo khoa, lãnh đạo phòng Công tác xã hội và lãnh đạo bệnh viện. Nghiên cứu viên ghi âm và ghi chép lại các thông tin thu thập được trong khi phỏng vấn và thảo luận. Kết quả và khuyến nghị: Kết quả định tính cho thấy một số yếu tố ảnh hưởng đến chi phí điều trị của người bệnh viêm phổi điều trị nội trú tại bệnh viện như: yếu tố tuổi, giới, số ngày điều trị, bệnh lý mắc kèm, mức độ bệnh, BHYT, ngoài ra còn có các yếu tố đến từ phía nhà quản lý như: chính sách của BHYT và bệnh viện, hoạt động dinh dưỡng trong bệnh viện, ứng dụng CNTT trong quản lý, giám sát bệnh, sự hỗ trợ từ CTXH, công tác chỉ đạo tuyến. Khuyến nghị tăng cương hoạt động của Khoa Dinh dưỡng tiết chế và phòng Công tác xã hội, vừa đem lại nguồn thu cho đơn vị vừa giảm bớt gánh nặng chi phí cho quá trình điều trị. Ngoài ra bệnh viện cũng cần hoàn thiện cơ sở hạ tầng để thực hiện kế hoạch triển khai bệnh án điện tử.
Analyze several factors affecting inpatient treatment costs for pneumonia patients at Pham Ngoc Thach Hospital, Ho Chi Minh City, in 2023. Methods: This is a qualitative study. The qualitative components were conducted using in-depth interview guides and group discussions for different target groups regarding the factors affecting the costs of pneumonia patients being treated at the hospital. These were developed based on a theoretical framework used to interview patients/their families, treating physicians, department heads, leaders of the Social Work Department, and hospital management. The researchers recorded and noted the information gathered during interviews and discussions. Results and Recommendations: The qualitative results identified several factors affecting the treatment costs of pneumonia patients hospitalized at the facility, such as age, gender, number of treatment days, comorbidities, disease severity, and health insurance (HI) status. Additionally, factors from the management side, including HI policies, hospital policies, nutrition services, IT applications in disease management and monitoring, support from the Social Work Department, and referrals from the hospital, were found to influence costs. Recommendations include enhancing the activities of the Nutrition Department and the Social Work Department to both increase revenue for the hospital and reduce the financial burden on patients. Furthermore, the hospital should improve its infrastructure to implement an electronic medical record system.
