The authors report a case of petrous internal carotid aneuvrysm in an 75 years old male. The clinic diagnostic of this disorder is difficult, it is extreme rare and presentation with otologic symptoms is unusual, not specific. These suggestive symptoms include hearing loss, pulsatile tinnitus and life threatening hemorrhage. The authors believe that, in petrous carotid aneuvrysm performance of a CT scan of temporal bone and carotid artery with intravenous contrast, contributes to the diagnostic. In addition, this report also demonstrates the usefulness of endovascular treatments for aneuvrysms of the petrous portion of the internal carotid artery, which are extremely difficult to treat by using an open surgical occlusion.
