Mô tả đặc điểm di truyền, lâm sàng và hình ảnh học của nhóm người bệnh Alzheimer có bất thường di truyền. Kết quả: Các bất thường di truyền ghi nhận được là APOE ε3/ε4, APOE ε4/ε4, APOE ε3/ε4 + APP, APOE ε4/ε4 + PSEN1 và PSEN2 với tỉ lệ lần lượt là 60%, 30%, 3,33%, 3,33% và 3,33%. Về phổ thần kinh nhận thức, nhóm APOE ε3/ε4 giảm nổi bật về trí nhớ, nhóm APOE ε4/ε4 biểu hiện rõ về giảm trí nhớ, tập trung chú ý và thị giác không gian, nhóm APOE ε4/ε4 + PSEN1 biểu hiện giảm nổi bật về trí nhớ, tập trung chú ý, điều hành, thị giác không gian. Về đặc điểm hình ảnh học, nhóm APOE ε3/ε4 biểu hiện nổi bật ở teo hải mã, nhóm APOE ε4/ε4 biểu hiện rõ đặc điểm teo hải mã và sang thương chất trắng dưới vỏ, nhóm PSEN2 biểu hiện chủ yếu teo não phía sau. Kết luận: Người bệnh Alzheimer có bất thường di truyền có tuổi khởi phát bệnh sớm, lâm sàng và hình ảnh học không điển hình và tốc độ diễn tiến bệnh nhanh. Trong đó, bất thường di truyền thường gặp nhất là APOE ε3/ε4.
To describe the proportions of genetic abnormalities, clinical characteristics, and neuroimaging features of Alzheimer’s disease with genetic abnormalities. Results: The genetic abnormalities recorded were APOE ε3/ε4, APOE ε4/ε4, APOE ε3/ε4 + APP, APOE ε4/ε4 + PSEN1 and PSEN2 with rates of 60%, 30%, 3.33%, 3.33% and 3.33%, respectively. Regarding the neurocognitive spectrum, the APOE ε3/ε4 group has a prominent decrease in memory, the APOE ε4/ε4 group clearly shows a decrease in memory, attention, and spatial vision, the APOE ε4/ε4 + PSEN1 group shows notable reductions in memory, attention, executive function, and spatial vision. Regarding imaging characteristics, the APOE ε3/ε4 group shows prominent signs of hippocampal atrophy, the APOE ε4/ε4 group clearly shows features of hippocampal atrophy and subcortical white matter lesions, and the PSEN2 group shows mainly posterior brain atrophy. Conclusion: People with Alzheimer's disease with genetic abnormalities have an early age of onset, atypical clinical and imaging findings, and a rapid rate of disease progression. Among them, the most common genetic abnormality is APOE ε3/ε4.
