Mô tả triệu chứng lâm sàng theo thang điểm triệu chứng liên quan đến sữa bò (CoMiSS) ở trẻ em tại Bệnh viện Nhi Trung ương. Đối tượng: gồm 44 trẻ được chẩn đoán dị ứng protein sữa bò ở trẻ em trước 2 tuổi tại Bệnh viện Nhi Trung ương từ tháng 01/2021 đến tháng 04/2022. Phương pháp: Nghiên cứu mô tả cắt ngang, tiến cứu. Trẻ được thăm khám lâm sàng, đánh giá các triệu chứng lâm sàng theo 5 triệu chứng của thang điểm CoMiSS và đưa ra tổng điểm CoMiSS. Kết quả: 44 trẻ được nghiên cứu có tuổi trung vị là 5,5 tháng tuổi. Triệu chứng da và tiêu hóa thường gặp nhất với tỷ lệ 88,6% và 72,7%. Theo thang điểm CoMiSS, triệu chứng da chiếm tỷ lệ cao nhất là mày đay (61,4%); trong triệu chứng tiêu hóa có 50% trẻ thay đổi tính chất phân theo hướng lỏng và nước hoặc táo bón, 9,1% trẻ nôn hoàn toàn. Triệu chứng hô hấp và toàn thân ít gặp, chiếm tỷ lệ lần lượt là 22,7% và 34%. Tổng điểm CoMiSS < 12 điểm chiếm 77,3%, điểm trung vị là 9 điểm, trong đó điểm thấp nhất là 6 và điểm cao nhất là 24 điểm. Kết luận: Thang điểm CoMiSS là thang điểm đơn giản, dễ sử dụng để đánh giá triệu chứng liên quan đến sữa bò dựa vào 5 triệu chứng phổ biến nhất ở trẻ được chẩn đoán dị ứng protein sữa bò. Trẻ có tổng điểm triệu chứng càng cao thì triệu chứng được đánh giá nhiều khả năng liên quan đến sữa bò càng cao.
o describe clinical manifestations according to the CoMiSS in children at National Children's Hospital (NCH). Subjects: 44 children diagnosed with Cow's milk protein allergy before 2 years old, at the NCH from January 2021 to April 2022. Methods: A cross-sectional, prospective descriptive study. Based on the five symptoms of the CoMiSS scale, children were examined and evaluated for clinical symptoms to estimate the total score of the CoMiSS. Results: The median age was 5.5 months old. Cutaneous and digestive manifestations were the most common, at 88.6% and respectively. According to the CoMiSS scale, urticaria was the cutaneous symptom occupying the highest percentage of 61.4%; while among children with gastrointestinal manifestations, 50% and 9.1% of those presented with watery diarrhea and/or constipation and vomiting, respectively. Respiratory and systemic symptoms were uncommon, accounting for 22.7% and 34%, respectively. The total CoMiSS Score under 12 points occurred in 77.3%, the median score was 9 (range 6 to 24) points. Conclusions: With the five most common symptoms in children diagnosed with Cow's milk protein allergy, the CoMiSS scale is a simple and effortless tool to evaluate cow's milk- related symptoms. Children with a higher total score suggest their symptoms are more like to be related to cow's milk.
