Ghép thận trên bệnh nhân suy thận mạn giai đoạn cuối do bệnh thận đa nang là một thách thức, việc chỉ định cắt thận đa nang trước mổ ghép thận còn nhiều tranh luận. Chúng tôi trình bày các trường hợp bị bệnh thận đa nang được ghép thận tại BVND 115 nhằm mục tiêu: Nhận xét đặc điểm lâm sàng, cận lâm sàng, chỉ định cắt thận đa nang trước mổ và kết quả ghép thận ở những bệnh nhân có bệnh thận đa nang được ghép thận tại BVND 115. Đối tượng và phương pháp nghiên cứu: hồi cứu mô tả loạt ca Kết quả: Có 8/313 TH (2,5%) bệnh nhân bị bệnh thận đa nang được ghép thận tại BVND 115 từ tháng 2/2004 đến tháng 8/2020. Tuổi trung bình của 8 TH bệnh thận đa nang: 57,21 ± 3,76 tuổi (52-68 tuổi), có 5/8 nam và 3/8 nữ. Chỉ định mổ cắt thận đa nang trước mổ ghép: 2/8 TH, đều do nhiễm trùng thận. Kết quả sau mổ ghép thận: Chức năng thận ghép tốt ở cả 3 thời điểm: xuất viện, 1 năm và 5 năm. Có 1 TH bị nhiễm khuẩn đường tiết niệu (NKĐTN) sau ghép thận do Pseudomonas putida, điều trị ổn định sau 7 ngày. Sống còn thận ghép: 1 năm là 8/8 TH (100%) và 5 năm là 8/8 TH (100%). Không ghi nhận biến chứng ngoại khoa, hoại tử ống thận cấp và thải ghép cấp. Kết luận: Tỉ lệ bệnh thận đa nang được ghép thận tại BVND 115 là 8/313 TH (2,5%). Có 2/8 TH cắt thận đa nang trước ghép đều do thận bị nhiễm trùng. Kết quả ghép thận tốt với sống còn thận ghép tại thời điểm 1 năm và 5 năm là 8/8 TH (100%) Có 1 TH bị NKĐTN sau ghép nhưng điều trị ổn định.
Kidney transplantation in patients with end-stage chronic kidney failure due to polycystic kidney disease is a challenge, the indication for polycystic nephrectomy before kidney transplant surgery is still controversial. We present cases of polycystic kidney disease undergoing kidney transplant at People's Hospital 115 with the aim of: Reviewing the clinical and paraclinical characteristics, indications for preoperative polycystic nephrectomy and kidney transplant results in these patients at People's Hospital 115 Subjects and methods: Retrospective description of case series Results: There were 8/313 TH (2.5%) patients with polycystic kidney disease who received kidney transplants at People's Hospital 115 from February 2004 to August 2020. Average age of 8 patients with polycystic kidney disease: 57.21 ± 3.76 years old (52-68 years old), 5/8 men and 3/8 women. Indications for polycystic nephrectomy before transplant include 2/8 cases, all due to kidney infection. Results after kidney transplant: Kidney function was good at all 3 time points: hospital discharge, 1 year and 5 years. There was 1 patient with urinary tract infection (UTI) after kidney transplant due to Pseudomonas putida, which was stable after 7 days of treatment. Kidney transplant survival at time points fisrt year is 8/8 TH (100%) and 5th year is 8/8 TH (100%). No surgical complications, acute tubular necrosis or acute graft rejection were recorded Conclusions: The rate of polycystic kidney disease undergoing kidney transplant at People's Hospital 115 was 8/313 cases (2.5%). There were 2/8 cases of polycystic nephrectomy before transplant due to kidney infection. There was 1 patient with UTI after kidney transplant due to Pseudomonas putida and stabilized after 7 days of treatment. Kidney transplant survival at 1 year and 5 years is 8/8 TH (100%).
