Đánh giá kết quả điều trị phẫu thuật u não ở người lớn tuổi. Phương pháp: Nghiên cứu hồi cứu mô tả trên 203 bệnh nhân u não người lớn tuổi phẫu thuật tại khoa Ngoại thần kinh - bệnh viện Chợ Rẫy từ tháng 1/2020 đến tháng 12/2020, đánh giá kết quả điều trị tại thời điểm xuất viện và sau 3 tháng. Kết quả: Tuổi trung bình trong nghiên cứu là 64,70 ± 4,06 tuổi, có 80 bệnh nhân nam (39,41%) và 123 bệnh nhân nữ (60,59%). Điểm Karnofsky trước mổ là 72,91 ± 11,56 điểm. Tỉ lệ bệnh nhân có bệnh nền là 38,42%. Triệu chứng thường gặp nhất là đau đầu (71,92%), yếu nửa người (29,56%) và buồn nôn hay nôn (24,63%). U màng não (43,35%), u tuyến yên (17,73%), u di căn não (8,87%) và u sao bào (7,39%) chiếm đa số. Có 82,76% bệnh nhân có kết cục tốt lúc xuất viện (GOS 4-5), 21,67% bệnh nhân có biến chứng phẫu thuật, tỉ lệ tử vong là 1,97%. Các yếu tố ảnh hưởng kết quả điều trị là điểm Karnofsky trước mổ và bệnh nền đái tháo đường. Tại thời điểm 3 tháng điểm Karnofsky là 77,46±9,64 điểm, tỉ lệ tử vong là 4,93%.
To evaluate the result of surgical treament of brain tumors in the elderly. Methods: We performed a retrospective descriptive study on 203 elderly patients undergoing brain tumor surgery at the Neurosurgery department of Cho Ray hospital from 01/2020 to 12/2020, evaluating treatment results at the time of discharge and after 3 months. Results: The average age in the study was 64.70 ± 4.06 years old, there were 80 male (39.41%) and 123 female patients (60.59%), Karnofsky score before surgery was 72, 91 ± 11.56 points. The rate of patients with comorbidities was 38.42%. The most common symptoms were headache (71.92%), hemiparesis (29.56%) and nausea or vomiting (24.63%). Meningiomas (43.35%), pituitary tumors (17.73%), brain metastases (8.87%) and astrocytoma (7,39%) accounted for the majority. There were 82.76% patients with good outcome at discharge (GOS 4-5), 21.67% patients had surgical complications, the mortality rate was 1.97%. The prognostic factors affecting the treatment outcome are the preoperative Karnofsky score and the diabetes mellitus. At 3 months, the Karnofsky score was 77.46 ± 9.64 points, the mortality rate was 4.93%.
