
Trocar setting is the first step of endoscopic surgery, which the surgeon has to know clearly. However, there might be some complications during this step, especially among patients with previous incision. The new method (set trocar guided by ultrasound) has been applied for endoscopic surgery among patients with previous surgery in Vietnam-Cuba Dong Hoi Friendship Hospital since November 2007. Aims: Limit the complications in setting initial trocar and value some other advantages of this method in endoscopic surgery. Study methods: Using ultrasound to estimate the depth of belly and identify the position to set the Veress needle. Then salty liquid were pumped into the belly through the Veress needle. The amount of fume was clarified by the ultrasound. Trocar was set by operating or pocking directly under the guide of ultrasound. Results: Six patients with previous incision were successfully implemented with the new technique. Cent percent (6/6 cases) was occurred smoothly with the amount of liquid (NaCl 0.9 percent) pumped was 2500ml. It took less than 20 minutes among five cases (83.3 percent). Trocar was set on previous incision in two cases (33.4 percent). The living indicators such as HA-SpO2-SpCO2 were in the normal limit, no complications were found.
