The authors applied Invasive Mechanical Ventilation (lMV) for treatment of severe respiratory failure of 37 patients in acute exacerbation COPD in ICU of 108 Central Military Hospital from January 2006 to July 2008. The results showed that Invasive Mechanical Ventilation was the method for survival treatment of severe respiratory failure of patients in acute exacerbation COPD; the rate of success was 62.17 percent. The complications of IMV were as followed: Nosocomial pulmonary infections with 70.2 percent, tachycardia with 56.67 percent, hemodynamic instability with 51.35 percent, conscious disorders (coma) with 43.24 percent, weaning difficulty with 37.8 percent, and pneumothorax 5.4 percent. Treatment of severe respiratory failure of patients in acute exacerbation COPD in ICU by Invasive Mechanical Ventilation was still difficult, that is, the long duration of ventilation (16.95 + or - 25.19 days), the long ICU stay (20.19 + or - 32.4 days), the high mortality: 37.83 percent.
