Nghiên cứu ứng dụng vi phẫu thuật thanh quản laser CO2 trong điều trị ung thư thanh quản giai đoạn sớm

Objectives: To evaluate the primary results of CO2 laser laryngeal microsurgery for laryngeal cancer at early stage. Methods: 50 patients with laryngeal cancer at T1, T2 stage, age from 42 to 72, were treated by CO2 laser laryngeal microsurgery, from Jan/2013 to May/2014, at ENT National Hospital, Ha Noi. Results: There were 48 males and 2 females, 35 cases were at T1aN0M0 stage, 8 cases T1bN0M0, 7 cases T2N0M0, the anterior commissure was involved in 5 cases, the two vocal cords were involved in 7 cases, the posterior commissure was involved in 4 cases. Tracheotomy was done in 10 cases (20 percent). The marginal security was good in all cases, no severe complications such as haemorrhage, dispnea was noted Gralunoma post-op was noted in 6 cases. One relapse in place was noted after 2 months and CO2 laser laryngeal microsurgery was done again. Conclusion: CO2 laser endoscopic surgery is initially effective and safe in the treatment of early glottic carcinoma not infiltrating the cartilaginous skeleton. The rate of tracheotomy was low. It could replace for open cordectomy's operation in some indications.
