Abstract

Aim: To study feasibility, safety and effectiveness of laparoscopic surgery (LS) in treatment of congenital duodenal obstruction (CDO) in children. Methods: Medical records of patients with diagnosis CDO without other gastrointestinal anomaly and generalized infection, with stable cardiovascular and respiratory function, operated by LS at National Hospital of Pediatrics from March, 2009 to July, 2012 were reviewed. Results: 15 patients were identified (9 boyss, 60 percent) with mean age at operation 11 days (ranged 3-63 days). All patients had vomiting, among them 11 patients (73 percent) had bilious vomiting, 4 patients- non bilious vomiting. Mean weight at operation was 2546 + or - 419g (min 1800g, max 3200g). In 10 patients, duodenal obstruction was caused by a web, in 3 -by annular pancreas and in 2 -by atresia with separation of the proximal and distal end. The most common site of obstruction was D2 (in 13 patients, 87 percent) and D3 (in 2 patients, 13 percent). Three trocars 3-5mm were used in all patients. Laparoscopic duodenoduodenostomy was carried out in 13 patients, duodenotomy and membrane resection In 2 patients. There was no complication and mortality, no case of conversion. The mean time to initial feeding was 5,6 days (ranged 2-12 days). The median postoperative hospital stay was 7,9 days (ranged 4-16 days). Conclusions: LS is feasible, safe and effective method for treatment of selected cases of CDO, even in neonates with low weight.
