Sarcoma cơ vân tuyến tiền liệt là một bệnh ác tính hiếm gặp nhưng diễn tiến nhanh, tiên lượng xấu. Chúng tôi báo cáo một trường hợp người bệnh 18 tuổi, được chuyển viện với chẩn đoán tổn thương thận cấp - áp xe tuyến tiền liệt. Lâm sàng của người bệnh gồm các biểu hiện như tiểu gắt, đau hạ vị 1 tháng nay nhưng không sốt. Thăm khám qua ngã hậu môn ghi nhận ở thành trước trực tràng có khối to, chắc, gồ ghề, không di động. Chụp cắt lớp vi tính phát hiện tuyến tiền liệt lớn, nhiều hạch quanh 2 bó mạch chậu. Chỉ số TPSA là 1,32ng/ml. Kết quả giải phẫu bệnh ban đầu là carcinoma tuyến biệt hóa kém. Mẫu sinh thiết được nhuộm hóa mô miễn dịch, kết quả cuối cùng xác nhận Sarcoma cơ vân tuyến tiền liệt, grad 3.
Prostate rhabdomyosarcoma is a rare but rapidly progressing malignant disease with a poor prognosis. We report a case of an 18-year-old male, referred to Binh Dan hospital with the diagnosis of acute kidney injury - prostate abscess. Clinical symptoms include symptoms such as dysuria and hypogastric pain for a month without fever. Digital rectal examination noted that there was a large, hard, rough, and non-mobile masses from the anterior rectal wall. Computed tomography detected a large, irregular shape of the prostate gland, with a lot of lymph nodes around the 2 pelvic vascular bundles. TPSA value was 1.32ng/ml. The initial pathological outcome is poorly differentiated adenocarcinoma. Then, the biopsy sample was immunohistochemically stained, the final results confirming Prostate Rhabdomyosarcoma, grade 3.
