Mô tả thực trạng thực hành các biện pháp trong gói dự phòng viêm phổi liên quan thở máy (VAP) của điều dưỡng. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 45 điều dưỡng làm việc tại đơn vị Hồi sức ngoại - Trung tâm Gây mê Hồi sức Bệnh viện Bạch Mai từ tháng 6/2023 đến tháng 6/2024. Kết quả: Các biện pháp được điều dưỡng tuân thủ thực hành đúng cao nhất là “nhận định số lượng và tính chất đờm” sau hút đờm (97,8%); “sử dụng nước vô trùng cho bình làm ẩm đúng và đủ” (95,6%), “không bơm nước vào NKQ/MKQ” khi hút đờm (95,6%) và “không bơm rửa miệng khi vệ sinh răng miệng” (95,6%); Các hạng mục thực hành có tỉ lệ thực hành đúng thấp nhất là “duy trì áp lực cuff 20 - 30 cm H₂O” (31,1%); vệ sinh tay “sau khi chạm bề mặt xung quanh người bệnh” (35,6%); vệ sinh tay “trước khi tiếp xúc người bệnh” (42,2%) và đảm bảo “vị trí thấp nhất dây máy thở là bẫy nước” (42,2%). Kết luận: Nghiên cứu cho thấy điều dưỡng chưa thực hành tốt các hạng mục về quản lý cuff, vị trí đặt bẫy nước đúng, vệ sinh tay trước tiếp xúc người bệnh và sau khi chạm bề mặt xung quanh người bệnh.
To describe the current practices of nurses on prevention of ventilator-associated pneumonia (VAP). Research subjects and methods: A cross-sectional study on 45 nurses working at the Center of Anesthesia and Intensive, Bach Mai Hospital was conducted from June 2023 to June 2024. Results: The highest adherence to correct practices among nurses were observed in "assessing the quantity and nature of sputum" after suctioning (97.8%), "using sterile water for humidifiers correctly and adequately" (95.6%), "avoid injecting water into the endotracheal/ tracheostomy tube" during suctioning (95.6%) and "avoid irrigating the mouth during oral care" (95.6%). On the other hand, the current practices were recorded with the lowest adherence rates, including "maintaining the tube cuff pressure adequately at 20-30 cm H₂O" (31.1%), performing hand hygiene "after touching patient surroundings" (35.6%) and "before touching a patient" (42.2%) and ensuring "the position of the water trap at the lowest point of the ventilator circuit system" (42.2%). Conclusion: The study revealed that nurses demonstrated poor adherence to practices related to cuff management, correct placement of the water trap, and hand hygiene both before touching a patient and after touching apatient’s surroundings.
