Abstract

Objectives: To assess the efficacy and safety of the initial results with Mini - PCNL and Modified - PCNL in the treatment of moderate size kidney and proximal ureteral stones. Patients and Methods: A retrospective analysis of 36 patients with moderate size (1025mm) kidney and ureteral stones who underwent Mini - NLPC (16 cases) and Modified NLFC (20 cases) between October 2012 and May 2013 was performed. Ureteroscope 9.5Ch and nephroscope 26Ch were used. Pneumatic or ultrasonic probes were used for lithotripsy. The ureteral stents and nephrostomy tube were place at the end of the procedure. Mean drop in hemoglobin, operative and fluoroscopy time, success rate, hospital stay and complications were assessed. Results: The average stone size was 18.6 + or - 5.0 mm (10-26 mm). The mean age of the patients was 45.6 (range 26-80 years). The duration of the operation was calculated as 47.6 minutes (range 20-80 mins). Mean Fluoroscopic screening time was 127 seconds (range 45-360 seconds). The patients were discharged after a mean hospitalization period of 1,37 days (range 1-3days) was archieved. The stone-free rate in all patients was 94.4 percent. No major complications such ashemorrhage, perfore or organic injury were noted during the operation or postoperatively. Conclusion: the initial results provide that Mini - NLFC and Modified- PCNL without modified instruments are a feasible, safe and efficacious treatment for moderate size kidney and proximal ureteral stone. This Technique no needs to purchase specialized equipment.
