Introduction: The Safety and effectiveness of endovascular fibrinolytic therapy administered within 6 hours after stroke onset in patients with acute ischemic stroke caused by intracranial occlusion of circulation is not fully evaluated. the authors evaluated the treatment effectiveness based on the modified Rankin scale score at 3 months and safety based on the mortality rate of intracerebral hemorrhage. Method: A longitudinal case series study method was applied on 94 ischemic patients. Results: The percent of good outcome patients (modified Rankin was ranked from 0 to 2) after 3 months of treatment is 44.7 percent. The percent of symptomatic cerebral hemorrhage patients is 8.5 percent. There are 19.1 percent of patients died after all. Conclusion: Endovascular fibrinolysis using rtPA therapy within the first 6 hours after on-set on the ischemic stroke patients in Vietnam is as effective and safe as what they are applying globally shown by many international researches.
